My NEI mode for changing gamemode, etc in the top left is gone. Wont show up, and NEI itself is there, itemson the right and options in the bottom left? Any ideas on why this would happen?

NEI not showing up?

Nei is able to be toggled. I believe the default key is O. ~Rawrz

Same thing actually happened to me. It's not a matter of pressing "o". The items on the right still show up, but the cheat mod/utility functionality is not showing up. I believe it has to do with the game mode upon world creation.

Yea, its not there in the top left, i see the items in the right, and i am in cheat mode. Maybe a clean update?
Then there may be a world setting set. Go to NEI's options where you can switch cheat/utility/recipe mode, click the "Global" button in the top right (it should now say "World") and then click the W button next to Recipe Mode.

I believe this may be related but we recently updated to v1.1.2 and same issue. but to add to this if you try and go into the creative nei screen it will crash your client
