Hmm, there's no obvious error in the crash log. The most likely problem is that your server's running out of memory. Modded Minecraft servers, especially heavily modded ones, generally have some memory leaks somewhere, meaning they require more and more RAM the longer they run. In my experience, a Direwolf20 pack server uses about 1.2GB RAM just to load the world, and you only have 1.5GB available to Minecraft.

Working within the system you have, I can think of 2 options: restart your Minecraft server every hour or so (there are ways to automate this, depending on your hosting provider) before the memory leaks crash the server, or remove enough mods that your memory usage is decreased. For the latter, you'd need to sacrifice some of the larger mods to make an impact; not just small things like Obsidian Pressure Plates, but some of the big mods like IC2. Even with removing mods, though, you'll need to restart the server occasionally to compensate for memory leaks. (The dw20 server I run for a few friends and myself has 4GB RAM, and needs to be restarted every couple of days)

As I said at the start, I'm not positive that your current problem is memory-related, but that would be my educated guess.

What are memory leaks and is there any way to fix them other than annoying restarts?

Memory leaks are I've been told because of how poorly written Java is and is basically when it does not release memory once it is no longer needed. Using up ALL DA MEMORIES.

I don't think there is a way.
