Seems more and more of this usage issue is happening, RZR0 said it has also something to do with database maintenance, I quote:
<RZR0> Adding the recipe will add the item to the SMW system <RZR0> But usages on pages that should refer to it have to have their cache updated for it to show <RZR0> it's a 2 step process <RZR0> that's why I generally do maintenance every so often <RZR0> The maintenance removes the entire SMW cache <RZR0> Then regenerates each page

Yes, the issue is that adding a new recipe doesn't automatically update all pages that would show this new recipe. That has to be done manually, either using the Refresh link, or by RZR0 clearing the cache.

Refreshing the page even Ctrl+F5 didn't seem to help much, but that was a while back, haven't seen the issue lately...

It generally wouldn't... You need to purge the page.

Exactly. Ctrl+F5 only clears your browser's cache, which you generally need to do after modifying your user CSS and such. The Refresh link on the page clears the server cache for that one page.

What do you mean by 'purge' exactly?
I have a fresh case: I just added the template to the page Portable Tank and using 'show preview' 2 items show up, but not the Hardened Tank, which recipe has been there a while...
Also if there's only 2 items listed it puts one on each side of the page... odd isn't it?

Update:
Well, today I created 2 pages (ME Toggle Bus and Inverted), the second uses the first item in it's recipe, upon saving the second, I refreshed the first and behold the item showed up in usage right away! same again when I added the shapeless recipe in the first page it showed up right away in the second page upon refresh.
So I loaded the portable tank pages and all the tanks are showing in the appropriate usage... go figure

What do you mean by 'purge' exactly?
This:

Refresh directs to ?action=purge
