This looks like some old config settings stayed where they shouldn't. Rename the Monster pack folder to something else, launch the pack to it gets redownloaded, and then copy the worlds from the old to the new Monster folder.

Monster 1.0.10 enabled Biomes O' Plenty, which uses dimension 20 for its Promised Land dimension. It looks like you might already have an Age 20 from Mystcraft, which is conflicting. Go to your configs folder, open "biomesoplenty\ids.cfg" with a text editor, and change "I:"Promised Land Dimension ID"=20" to something else, like "I:"Promised Land Dimension ID"=99". If you already have 99 Mystcraft dimensions, try a higher number.
